New Appointment Information

We would like to take this opportunity to welcome you to our practice and acquaint you with our office procedures. In order for us to efficiently and cost effectively help you in your infertility evaluation and treatment, we request for detailed information that will allow us to understand your particular needs and be better prepared for your visit.  Please input your information into our secure patient portal at least 3 days prior to your initial appointment.  You may access the patient portal by obtaining a user ID and password from our receptionists. You will need a separate user ID and password for the husband and the wife in order to enter and obtain information in both records.

Once in the patient portal we ask that you:

1.       Complete the patient registration and medical history forms.

If you have consulted with other physicians or practices regarding your infertility, please request that copies of your records from each physician be sent in advance.

1.       For the wife, specifically request reports for any previously completed hormonal testing (such as Prolactin, TSH, day 3 FSH, day 3 estradiol, day 10 FSH, testosterone, DHEAS, fasting glucose, 2 hour glucose or fasting insulin levels), Clomiphene Citrate Challenge test (CCCT), a saline sonogram (hysterosonogram) with pictures, hysterosalpingogram (HSG) with the actual x-ray films, prior obstetrical records, immunization records specifically for rubella (German Measles) and varicella (Chickenpox), laparoscopic or hysteroscopic surgery, and any pelvic surgery. X-ray films (many are now digital on a CD) should be obtained from the radiology department where they were performed. Operative reports may need to be obtained from the hospital but are usually available from the physician’s office. Records regarding any treatment such as clomiphene citrate, letrozole, gonadotropin (FSH) injections, intrauterine insemination (IUI) or in vitro fertilization (IVF) should also be obtained. Please be aware that records need to be requested from each physician/clinic that you have seen (physicians cannot forward records that they have received for review from other physicians or clinics) and need to be requested specifically for both the husband and the wife (usually require separate consent forms and requests).

2.       For the husband, please include copies of semen analysis reports, insemination preparation reports, urologic examinations, scrotal ultrasounds, any hormone testing (such as testosterone, prolactin, TSH and FSH) and any pertinent reproductive surgery (hernia repair, vasectomy, vasectomy reversal, etc).

·         Our office in Sandy is open from 8:00 am to 5:00 pm, Monday through Friday for appointments. New consultations are also available at 7 am and on Saturdays if needed. Weekend care is provided for treatment procedures.

·         Photo ID will be requested at initial appointment in order to comply with Red Flag Rules

·         No lab work or other procedure is included in the basic charge for the initial consultation.

·         We are located at 10150 S. Petunia Way (10150 S 1405 E). We are directly south of the Sandy City Library. There is abundant parking and easy access.

·         Completing the registration form, patient portal questions and obtaining all pertinent medical records in advance will allow our physicians to review your records prior to your arrival. This will enable us to provide the best advice possible for your particular medical situation during your initial consultation. If the registration form and all pertinent medical records are not received at least 3 days prior to your appointment, we recommend that you reschedule.

·         If you are unable to make your scheduled initial appointment we require a minimum of 72 hours notice or a $75 charge will be assessed.

·         Payment is expected at the time services are rendered. If you have insurance benefit we will be glad to bill your insurance.

PATIENT PORTAL INSTRUCTIONS

At Reproductive Care Center we attempt to make communication as simple as possible for you.  You may have test results, which you would like to know or you may be in a treatment cycle and need results of daily tests and instructions.  Therefore, we are implementing a system whereby you can access some of your results and instructions from any computer at a time that is convenient for you via our secure Patient Portal.  You can view your future appointments, request simple changes to your demographic information, and request information via questions to our staff. Below are instructions for accessing your information.  The front office staff will give you a username and assign a password so only you and your spouse can access this private information. You may change your password once you have successfully logged in for the first time. Please let us know if you have any questions or concerns.

For our new patients:
It is very important that you complete the health questionnaires, for both the husband and the wife, and demographic information contained in the Patient Portal.

You will enter the patient portal via our clinic’s website.  You will need a separate user ID and password for the husband and the wife in order to enter and obtain information in both records.

  1. Click on https://secure.fertilitydr.com/patient/
  2. Once the login appears, enter the username and password you were given then select login.
  3. Once you have successfully logged in, a Terms of Use agreement will appear.
  4. Read through this and choose either agree or I don’t agree. If you do not agree you will not be able to use this portal.
  5. After you have agreed, the next page is a basic demographics page. Changes are submitted to the administrative staff and will not be reflected immediately.
  6. Complete all of the Demographic Information (click "next" at the end of each page to move forward).
  7. After completing the Demographic Information, click on "Questionnaire" on the Patient Appointments page.
  8. Next click on "Patient Evaluation" and complete all of the questions regarding your medical history contained therein (click "next" at the end of each page to move forward).
  9. Click on "Profile" and complete all of the information therein including your insurance information (click on the dark blue tabs for each section).
  10. All options available to you are listed on the left side. The degree of access to your medical record will be determined by the clinic.
  11. Please be sure to complete the information for both partners via a separate "Log on" with a different User ID and Password for each.
  12. When finished, click on "Log off"
